Primary Care Networks
We are part of Salisbury Plain PCN, PCNs are a group of GP practices working together with a range of local providers to offer more personalised and coordinated health and social care for patients.
Salisbury Plain PCN compromises of Avon Valley Practice and Sarum Health Group. We will be working together to provide appointments during the weekend, to improve mental health care provision and to further collaboration with other organisations so we can expand the range of services we provide for patients.
