Primary Care Networks
Primary Care Networks (PCNs) are being established across the country. They are groups of GP practices working together with a range of local providers to offer more personalised and coordinated health and social care for patients.
Sarum Health Group (comprising Millstream Medical Centre and surgeries in Durrington and Larkhill) is pleased to announce that we will be part of Salisbury Plain PCN from 1st April 2020. This new PCN shall allow us to be innovative, creative and sustainable within the ever changing NHS. Our PCN will be led by Dr Chet Sheth who is a GP Partner at Sarum Health Group. Salisbury Plain PCN compromises Avon Valley Practice and Sarum Health Group.
We will be working together to provide appointments during the weekend, to improve mental health care provision and to further collaboration with other organisations including pharmacy so we can expand the range of services we provide for patients.
